Blue Hill Memorial Hospital's Transitional Level of Care (TLC) Program is designed to meet your individual rehabilitation needs and enable you to return home and/or to work.
Our program also offers compassionate end-of-life care which provides support and comfort to patients and their families.
What is the Transitional Level of Care (TLC) Program? Transitional Level of Care (TLC) Program allows your physician to "swing" your level of care from acute medical care to skilled rehabilitation.
What programs do you offer? A full range of services is offered on an inpatient basis. Therapies include, rehabilitation after a stroke, hip or knee replacement surgery, IV medication therapy and strength training after a prolonged hospitalization. This program is complimented by an activities program.
As a patient from another hospital, may I still receive rehabilitation at Blue Hill Memorial Hospital? Yes, if it is a planned surgery, we can coordinate your rehabilitation with the approval of your BHMH physician.
How long will I need to stay in the program? You may remain in the program for as long as you have skilled therapeutic goals that need to be met. Once these goals have been reached, you will be discharged from the program.
Who pays for this program? Transitional Level of Care (TLC) Program is often covered by Medicare and Medicaid, as well as many private insurance plans. If you have questions about insurance coverage, our social worker is available to assist you.
SERVICES AVAILABLE at BHMH
Nursing: Provides support, care and education during your recovery.
Nutrition Therapy: Offers nutritional support and education to promote optimal nutritional health.
Physical Therapy: Works with you to improve endurance and resume life activities such as walking, exercising, improving balance, strength and flexibility. Therapists will also educate you and your family on safety and mobility.
Occupational Therapy: Works to restore independence in daily living after an illness, injury, or surgery. Provides education and training to accomplish tasks to avoid pain and fatigue. Therapists will assess the need for assistive devices and provide training.
Respiratory Therapy: Teaches breathing techniques and exercises to promote optimal breathing.
Social Work: Assists you, your family and/or significant others with your individualized goals including discharge planning, personal resources and financial needs.
Speech Therapy: Assists patients with interpreting and remembering written and spoken statements; expressing thoughts through speaking, writing, and facial expressions and promote safe swallowing.
