Physician's Foundation for Health Systems Excellence has awarded BHMH a $219,000 grant for a project entitled "Improving Care for Patients with Chronic Conditions in Rural Maine". The grant period is January 1, 2006 to December 31, 2007.
The goals of the project include 1) To enhance the use of evidence-based medicine and best practices for patients diagnosed with one or more chronic medical conditions and 2) Enhance patient and provider access to community-based services and patient support services.
The target population of the grant will include all current and new patients of Peninsula Primary Care who are diagnosed with one of more of the following chronic conditions: high cholesterol, hypertension, congestive heart failure, diabetes, obesity, COPD, and major depression.
According to Dr. Phil Elkin, Project Director, "The goals of the grant mesh very well with the mission statement/vision statement developed in the strategic planning committee to measurably improve the health of the communities we serve."
The grant is a collaborative effort with the Public Health Research Institute, EMHS, and Institute for Medical Improvement, and Healthy Peninsula Project.
Says Dr. Elkin, "I think this project help jump start our efforts at maximizing the quality of our outpatient care for multiple chronic conditions and give us new tools to achieve this in an ongoing way. It will provide our patients and our community with improved resources for care and prevention. Finally, it will further our networking with our community, the EMHS system, and the state." |
