Blue Hill Memorial Hospital and its affiliated physician offices in Deer Isle, Castine, Bucksport and Blue Hill want to help you prevent chronic disease.
We will focus on identifying 5 risk factors for chronic disease:

© High total cholesterol and/or low HDL (healthy) cholesterol
© Smoking
© Physically inactive
© Overweight
You will receive your own chronic risk & behavior card. You and your nurse educator will work together to decide where you feel you can make changes to improve your health. You will learn how to:
© Control your blood pressure
© Manage your cholesterol
© Control your weight
© Eat a healthier diet
© Manage stress
© Kick the tobacco habit
© Take your medications properly
© Be more active
By participating, you will:
© Know and understand more about what is a healthy lifestyle
© Receive a personal plan for making healthy changes
© Receive educational materials to take home
© Receive referrals to community resources
This is part of your regular doctor's visit. Both your doctor and nurse educator will work with you to determine your follow-up plan. This may include:
© Referral to a dietitian (food expert)
© Smoking support group
© Behavioral health counselor
© Other members of your health care team
